No date--just a pub number of 6361470, no revision number.  The O&A was a
"work in progress".  Periodically, you were sent a bunch of looseleaf
3-ring sheets to add or exchange in the various sections.  I discarded
nothing--just moved it to the back of each binder.  There's only one CGA
display reference, and, as far as I can tell, no updates.
