Where to obtain diskette labels?

Cool, can you tell me what the Avery "number" is for them?  (should be 
on the box)  33 to a sheet is not #5197 (which are quite exorbitantly 
priced for some crazy reason) but if you've used something that works 
for you, I'd like to know the number so I can duplicate.

On a related note, how about write-protect tabs?
