Likewise, I have never had a problem - for me they Just Work (tm)   -  
Now to admit my dirty little secret...  Just use the TX/RX/GND Lines, 
and tie RTS-CTS, and DSR-DTS-CD (At each end of the cable)  - Hardware 
handshaking never quite works the way you expect it to!
