"A"), & you sould have gotten it (99/4's are considerably rarer than
99/4A's). 
 I remember it as a flat plastic sheet, but will look again next month. BTW
 it did also have a small box , either sound or voice. 
You're thinking of the Odyssey2 keyboard that you also saw.
